Sports Physicals dated after May 1 can now be turned in for next year. Three items are required: Sports physical dated after May 1, Parent-Athlete contract, and NSAA form. All are conveniently located here:

Please join the DASH Sports Medicine page. Parents already have the app for the sport their daughter is in, so there's no need to download anything else.
